Netview: Unable to see controls in netview shader builder pages

Symptom

When trying to use any of the Shader Builder pages, you do not see any check or text entry boxes, and the following popup error occurs:

 An error has occurred in the script on this page.
 Line: 460
 Char: 1
 Error: Object doesn't support this property or method
 Code: 0
 URL: mk:@MSITStore:...........(etc)

Cause

You are missing some ActiveX controls. Specifically those having to do with Microsoft Forms 2.0.

Solution

These controls come with Microsoft Office, and Microsoft Visual Basic, and can be installed from there. Alternatively, you can install the ActiveX control pad, which will give you Forms capabilities, from Microsoft ActiveX Control Pad (http://msdn.microsoft.com/library/default.asp?url=/library/en-us/dnaxctrl/html/cpad.asp)
