Bones are flipping to another position/orientation than stored in the clip

Symptom
After aplying the action clip to the same model, the bones are flipping to another position/orientation than stored in the clip.
Cause
This may be due to the fact that you have rotated the bones (FK) instead of moving the effector (IK).
Solution
You must change the blend mode to FK because the IK method is dominant by default. Therefore, on the first bone of the chain set the kinematic chain > FK/IK blend to FK.
